Keeping yourself accountable is truly underrated. When you rationalize why you didn't do something previously it becomes abstract and slips your mind pretty easily and this creates this cycle of not doing what you should be doing.
I use a white board to write down my daily/weekly habits that I hold myself accountable for. This helps because I can see it daily and weekly and can now quantify the actual effort I put in, versus allowing it to fade. This helps keep you accountable, and can be a strong driver to promote change.
