#247 Personalised Care Planning in Practice

We have an emotional one for you this week where I share my experience of creating a care plan for my youngest who has type 1 diabetes.   It's funny because, on one hand, I say I am absolutely fine, and then when I talk about it, I find it incredibly tough and end up crying, and I’m not really a crier.  Maybe it's the pent-up stress of it all because it's relentless.   I’m sharing my experience because I straddle both worlds. I am a health care professional, and I am a mum and a carer to my daughter with a long-term condition. I know all services are overwhelming, so I do not want to bother anyone. I also know there are online resources there, but I also want to talk to some face to face or at least a phone call with someone who kind of knows me. BUT…..  we do have a tremendous amount of support. I just need to ask for it!   In this podcast, I share:
