Dr. Robert Liston...he was an accomplished surgeon in the 19th century, a time when anesthesia didn't exist, nor did universal precautions and hospital cleanliness.  His success rate for surgeries was twice as high as most, but there were a few questionable surgeries, a 300% mortality rate for one surgery and one man lost a testicle during his leg amputation...
